Populate Lookup field from URL

Populating fields from the form URL is easy. All you have to do is add a parameter to the end of the URL containing the field ID and the value you would like to set inside it. 
In this version we’ve added the ability to also populate a Lookup element, and we can all thank Oded Breda for that! good idea, Oded!
All you have to do is add the following parameters at the end of the page URL and specify the Id of the record and its text. like this:
?fld4-value=11111&fld4-text=testing
