Resized modal windows in my submissions

When a user creates a new entry in the “My submissions” page or duplicates a selected entry, a modal window automatically opens to allow him to enter/edit data. Till now, however, there were times when the form was too wide or too high and we found that the window was not large enough, so in this version we have made the modal windows for Adding a submission or Duplicating a submission much bigger.
